Pay Stubs (4 most recent consecutive)
Most Recent Tax Returns
Unemployment Claim for Benefits
Worker's Compensation/Disability
Social Security Award Letter or Computer Print Out
Pensions/Retirement Fund
Any Other Income
Bank Statement for Checking and Savings Accounts
Certificate of Deposit
Mutual Funds/Stocks/Bonds
401K/IRA/Other Retirement Accounts
Fair Market Value of any Real Estate (if property was sold provide settlement sheet)
Life Insurance
Savings Bonds
Any Other Assets
Social Security Card and Birth Certificate
